Pharmacist’s Role in Management of Patients with Dementia and Delirium

As critical members of the interdisciplinary team, pharmacists are responsible for ensuring safe and effective pharmacotherapy for patients with dementia. There are many elements of this role, including reviewing medications regularly; discussing medication therapy with the patient, the healthcare team, and family or caregivers; and providing recommendations for treatment of dementia and associated symptoms. This lesson will review how pharmacists can play an active, collaborative role in the care of patients with dementia.
